Preparing for Appointments

Appointments

For your initial appointment, please arrive 10 minutes early in order to fill out our patient registration forms. You can also fill out the pre appointment registration form online. Please ensure that;

• You have a valid referral letter from your GP or referring Specialist

• You bring your Medicare Card and Private Insurance Details.

• You bring copies of all test results including pathology, audiology, imaging ( incl; Xrays, Ultrasound, CT, MRI, PET scans ).

• You bring details of your current medication list and allergies to medications

• Please bring any other information you may find relevant to your consultation

Cancelling or rescheduling your appointment:

• If you need to cancel or reschedule your appointment, please ensure that you do this as early as possible so that we make alternative arrangements for you and ensure that you receive the best care possible.

• Please do not hesitate to contact us should you have any concerns or questions regarding your upcoming consultation with A/Prof Melville da Cruz.
